JOB DESCRIPTION: Works collaboratively with all health care providers to optimize patient care. Responsible for the overall functions of the dispensing duties, preparation of IV admixture and keeping up-to-date knowledge in the field to function as a drug consultant to all members in the medical profession. Coordinates activities of the Pharmacy Department in accordance with the established policies and procedures of the hospital’s Pharmacy. Assists the department in providing pharmaceutical services to include advising providers pertaining to drug usage and control within assigned hospital. Provide specialized pharmaceutical services to improve drug usage and therapeutic outcomes including advising providers on issues concerning drug therapy, the inherent toxicity of drugs and side effects, as well as assisting in the prescription of appropriate doses. Coordinate the activities of assigned pharmacy technicians engaged in the preparation, labeling and delivery of medications and other pharmaceutical supplies in accordance with provider’s prescriptions: ensure compliance with federal, state and local laws and regulations pertaining to the dispensing of drugs and controlled substances and the maintenance of required records. Maintain liaison relationship with medical and nursing staff, provide timely information pertaining to pharmaceutical supplies, drug usage and compatibilities, state and federal regulations regarding drug controls, and Joint Commission standards. Prepares TPN, chemotherapy and other IV products. Conducts and evaluates medication histories, assess compliance and suggest modifications so as to achieve desired outcomes. Mentor/Supervise pharmacists in training including but not limited to pharmacy interns, activities should include both practical and didactic. Utilizes independent judgment to decide what to do to assemble facts, determine variations from standard procedures, or plan other action to insure the operation of the pharmacy. Suggests improvement in work methods. Individualize medication regimens using sound principles, accounting for pharmacodynamic and pharmacokinetic variations in drug absorption. Anticipates and contributes to management and professional committees and special meetings at hospital, community, etc. Keeps informed on developments and trends inside and outside the hospital which relate to assigned functions. Participates in hospital in-service programs when required. Requirements Preferred Education Bachelors of Science in Pharmacy. Or Doctorate of Pharmacy Degree. Preferred Experience Three years of hospital experience preferred or a demonstrated ability to achieve positive patient outcomes in a similar environment Required Certification/Licensure Current New York State License. Special Skills Ability to communicate clearly in oral and written form. Extensive knowledge of drug products, medical therapeutics, pharmacokinetics, and disease states. Ability to work with information systems, automated and technical equipment.
